The Use of a Representative Form [IMM 5476]
Purpose: For appointing a representative for doing business on behalf of the applicant and the applicant’s family members
Document Requirements:
- The applicant would need to provide this document only if:
- The applicant indicates that they are using a representative or,
- A consultant completes the application on behalf of the applicant via a consultant portal
- The principal applicant would need to sign the document along with the applicant’s spouse or common-law partner (if applicable)
- Each accompanying dependent child of 18 years of age or older, would also need to complete and sign their own IMM 5476 forms
- However, they would only do this if the representative is also conducting business on their behalf
The Individuals who would need to submit this documentation:
- The principal applicant
- The applicant’s spouse or common-law partner (if applicable)
- The applicant’s accompanying dependent children aged 18 years or older
